Patients paying through insurance will need to provide their insurance information through Headway, which handles insurance billing. You can set up a profile and request an appointment with me here.
New patients can also request an evaluation using the appointment widget on this page. For cash pay patients, evaluations are $450 and scheduled for one hour.
After an appointment is requested here and your time is confirmed, you will receive several e-mails, depending on how you requested an appointment: an appointment confirmation, a link to sign up for the patient portal, a notification that there are intake forms pending, and an invitation to provide your insurance information through Headway.
Existing patients can schedule appointments using the patient portal or through Headway.
